Important documents

We are so excited to welcome your child on our summer programme. To complete their application, please download the GP form template shown on the right and send it to your local GP for completion. We are also providing a template to be sent to a specialist if your child is seeing one.

These forms are required in order to ensure that we have a comprehensive medical record on file for your child that is specific to the conditions of camp to ensure that we are able to care for them to the best of our ability, and to give them the safest and best time possible!

Once the form is completed, you can upload it directly as part of the application process. If your GP needs a little more time, you are welcome to submit the rest of the form first and email the completed GP document to joanna@fzy.org.uk as soon as it is ready.
